Cord blood can only be collected at the time of your baby’s birth. It is therefore important to make the decision as soon as possible before the birth of your child. This will allow you time to discuss the procedure with your medical caregiver and ask Cryosite any questions you may have.

If you would like to take advantage of this service, you should return your completed documentation to Cryosite as soon as possible so that we can ensure a collection kit is sent to your hospital as soon as possible prior to your due date. Remember many babies arrive earlier than expected!

The following information explains what you need to do to arrange for the collection and storage of your baby’s cord blood with Cryosite.
1. Obtain the enrolment forms: These can be downloaded directly from the web, or sent by mail, fax or email. Simply do one of the following:

  1. Download the forms directly from our Web site
  2. Request a free information booklet to be sent by email, fax or mail using our online request form
  3. Request a free information booklet to be sent by email, fax or mail by calling 02 9420 1400

2. Read all the information carefully: It is important that you understand exactly what is involved in the process of collecting and storing cord blood. If after reading the information provided, you have any questions, do not hesitate to call Cryosite on 1800 220 410.

3. Sign the Storage Agreement: Once you are satisfied that you wish to store your child’s cord blood with Cryosite, you must sign the Cryosite Storage Agreement (Appendix 1).

4. Speak to your doctor: You will need to confirm with your doctor that he or she is willing to collect the cord blood at the time your baby is delivered. You and your doctor must both sign the Consent for Collection of Cord Blood (Appendix 2).

5. Complete the Medical History form: You must complete the mother’s Medical History form (Appendix 3). This enables Cryosite to assess whether there is a risk that your child’s cord blood could be infected and therefore ineligible for storage. The questions in the form are very similar to those that must be answered when a person donates blood.

6. Complete the Payment form: Please select your preferred payment option and fill in the payment details (Appendix 4) and the Direct Debit form (if elected) (Appendix 5).

7. Send the completed Storage Agreement, Consent for Collection, Medical History, Payment Option form, Direct Debit form (if elected) and payment to Cryosite:

8. Once we have received and processed your contract, you will receive confirmation that a collection kit has been dispatched to the delivery suite of your nominated hospital. You will also be provided with a 24 hour, toll-free number. This is the number for the Cryosite Cord Blood Call Centre which you will need to call after the birth of your baby to arrange for the pickup and delivery of the cord blood to our laboratory for processing, cryopreservation and storage.
