Our 3 step process
Arranging for the collection of your child's cord blood is an easy and hassle free process with Cryosite.
1. Complete the Cryosite Cord Blood Storage Agreement
Download and complete the Cryosite Cord Blood Storage Agreement, including the mother's medical history information and the Doctor's consent area. If your Doctor or midwife does not consent to collect your child’s cord blood please call us 1800 220 410.
2. Send Cryosite your completed Agreement and receive the Cord Blood Collection kit
Fax completed forms to 1800 220 410 or post to 13a Ferndell Street, South Granville, NSW 2142. Completed agreements can also be emailed to . Cryosite will check and confirm receipt of your Agreement then send your unique Cord Blood Collection kit approximately 4 weeks before your due date. If your baby is due sooner than this please call us because we can dispatch your Collection Kit overnight if required.
3. Bring your Collection Kit to the birth, and call 1800 220 410 for courier pickup
It is important that you bring your Collection Kit with you to the hospital for the birth of your child. The cord blood and maternal samples are collected by your obstetrician at the time of birth and placed in a temperature controlled shipping container and sealed.
You or your partner must then notify our specialist courier who is on call 24 hours a day, 7 days a week, 365 days a year by calling 1800 220 410. You (or a family member) will be asked to sign a packing slip and consignment note by the courier, releasing the sample for shipment to Cryosite.
Cryosite will then take care of the rest!
